New Patient Checklist

Once you make your appointment, we will email you a link to complete your patient paperwork online.  We kindly ask that you complete this prior to your appointment. If you would prefer to complete this in-office, please arrive 15 minutes prior to your appointment so we may assist you with your paperwork without delaying your appointment.

What to bring to your appointment:

• Patient Paperwork (completed online)

• Insurance card

• Photo ID

• Your glasses or glasses prescription

• Your contact lens prescription or boxes (if applicable)
